I've had this once with my SB900. At a wedding which was very annoying.
I've read that you can raise the ISO to help with this, but I've never tried it. Apparently it's more common at ISO s as low as 200. But not so much of a problem at 400. Like I say, not sure how accurate this is as I've not done it myself, but I've read that it can help.
I've also read that this may happen to stop damage being caused??
Having a back up flash may help, but can be expensive.
