Unwanted Grain

Some months ago I received criticism that my photos seemed grainy. On close examination I agreed but I just couldn't see why they were grainy. When II processed them using Camera RAW they were fine but as soon as I saved them in Photoshop, there was the grain. Can anyone advise me whether there is something I should be doing when it comes to saving the files?I have experimented a bit and wonder if it is anything to do with the 'baseline options' when I go to save as jpeg. I have had it on 'progressive' but today I saved it using 'optimised' instead. Can anyone explain these options?

"Since you're shooting for the highest image quality possible, I would choose baseline standard. This will make the least amount of changes to your image. Optimized compresses the image to save space, and progressive loads the image one line at a time, as opposed to loading the entire image at once."

But I would be surprised if it would make that much of a difference its more likely that you are over sharpening the image
