I usually include a lens pen and blower... as these are helpful if I need to clean my lenses during the day... e.g. Rain spots get on the glass.

I tend to shoot outdoors a lot, wildlife, macro mostly, so I carry a small torch to help lighten dark areas, a heavy duty plastic bin liner, so I can sit or kneel on and not get wet or to dirty.
I also make sure that I carry around a few business type cards with e-mail and website address in case I need to pass them on or meet a fellow snapper that I wnat to keep in contact with.

Planning is key - if you are doing documentary-type hard news photos that require fast reaction on your part, the less kit you carry, the better (but don't forget the extra batteries and memory cards). You will very rarely see news photographers carrying their bags around with them! Everything they need is carried on person, which gives them more mobility and allows them to get crucial shots.

If you are doing landscapes and other photography where time is not of the essence - ie. you will not miss the million dollar shot because you are changing lenses and so on - then carrying multiple lenses, tripods, filters, batteries, remote triggers, reflectors, etc is not an issue.

Canon 400D
Finepix F10 compact
18mm-55mm
55mm-250mm IS
18mm-135mm IS
Remote, spare batteries, spare CF & XD cards, Butterfly sensor cleaner
Slik Pro tripod
All in a Lowpro Backpack camera bag.The type that swivels onto your front and you get access through the back of the pack, makes a tray that is attached to you and is easy to get at stuff.

Well, what I take depends on the time of year, time of day, if the sun is shinning or not or if it's raining. I usually have a macro lens, 70-200mm and 14-24mm or 24-70mm lenses. Sometimes I take the tripod but not always and it depends on what may be around to shoot that day. I usually have a lens-cloth and a towel, a drink and something to eat. Spare battery is always in my jacket pocket plus a lens pen and memory card are always in my zipped trouser pocket. Other lenses are added or not as required and flash is the same.

John - I'm like Carl, never leave without a spare battery, CF cards, polarising filter, cleaning cloth, small notebook and pencil. Everything else Lenses, other filters etc. just take approprate to what I'm going to shoot. Usually get everything into a small shoulder bag, which fits into a rucksack if going hill walking.

I always have my camera instruction manual in my small kit bag in case i forget how to switch my camera on !
I also have a small torch/duster/polarizing filter/sync lead/spare battery/cf cards and 580 EX11 speedlight with a couple of difusers.
Oh and i ALWAYS remember to look behind me when out shooting<<That is by far the best piece of advice i read awhile ago and i implement to the full.....
