It definitely sounds like its graphics card related. Most modern cards have two outlets on the back of them. Have you tried swapping the cable over? My dad had a similar problem every time he booted the PC up. Since I suggested he swap the cable over to the other port, he's not had an issue.
Taking the card out and reseating it as others have suggested may be worth a try. Also, make sure that any additional power cables inside the pc are connected to the GFX card. If I remember rightly, mine has two cables powering the card.
The only other thing I can think of, is that your PC is going into power saving mode. Mine sometimes goes into "AiNap" which powers down devices to save energy.
